2016 Fredrick J. Mummert Distinguished Service Award -  Nomination Form

Nomination Guidelines

This award is presented to someone who personifies professional excellence and whose long-term dedication has enhanced PLNA's services, standards or image.  Candidates must have provided a minimum of twenty years of outstanding leadership and service to PLNA and/or PLNA Foundation (formerly PFOH), received the PLNA Lifetime Member distinction and served PLNA at the Chapter, Committee and Board levels.  

This award is differentiated from the PLNA Hall of Fame Award in that it is meant to recognize an outstanding "lifetime” commitment to PLNA and the industry. This award does not take into consideration other contributions to the community or the recipient’s current profession.  It is not required that this award is presented annually.  

Nominations may come from any Board of Directors member in good standing. Please note prior award recipients:

Additional information is necessary to properly evaluate each nominee.  Please provide as much information as you can regarding the nominee's contributions in the following areas:

  • Industry
  • Association

Years of service will be verified by PLNA Staff. 

Please explain your nominee’s contributions to the industry. Include speaking engagements, support of extension, sharing of horticultural information, career recruitment, and promotion of professionalism.

What contributions has this person made to the association? Consider committee service, Board of Director service, Executive committee service, committee chairmanship and vice-chairmanship, and overall support of Association services.
Now we need your information.  Be prepared to discuss your nomination at the June Board of Directors meeting.
